Here’s how it works:

1. Your dentist will present a treatment plan and offer a suitable payment plan.
2. You will decide on the payment plan details with your dentist, including the deposit, amount and frequency of payments.
3. Your dentist will submit the completed form to DentiCare to establish your DentiCare payment plan.
4. You’ll begin your treatment straight away and start making payments to Lightwood Dental.
